OSHA issues fines for Stage Manager killed in blast

2016-02-03 -Last summer we reported about an incident in west Texas where a Stage Manager was killed when a pyrotechnics storage bunker for an outdoor show exploded  (http://theatresafetyblog.blogspot.com/2015/08/fireworks-storage-building-explodes-and.html)

OSHA has now investigated the incident and issued a statement:  “A 21-year-old worker, with three months of work experience under her belt, lost her life because Texas Panhandle Heritage Foundation failed to provide appropriate training and protective equipment to workers handling pyrotechnics.”

That was the assessment of Elizabeth Linda Routh, OSHA’s area director in Lubbock, Texas, after a young worker was fatally injured last summer while taking an inventory of the pyrotechnics for an outdoor musical production at the Palo Duro Canyon State Park in Canyon, Texas.

After an inspection following the fatality, OSHA issued six serious citations for failing to train workers on the use of explosives, provide fire retardant clothing, perform a hazard assessment and develop a written hazard communication program.

Proposed Penalties: $42,000

Texas Panhandle Heritage Foundation is a performing arts employer.

Although $42,000 may not seem like much for a human life, this assessment of the situation opens the door for civil litigation so the victim's family can pursue further compensation.

Burn Survivor from The Station Nightclub fire speaks-out for the NFPA

Nightclub Fire in Romania Kills 27, Injures 170

2015-10-31 - Bucharest, Romania - An late night fire in a basement nightclub claimed 39 lives, injured over 190, and was completely predictable and preventable.  A heavy metal band Goodbye to Gravity gave a free Halloween concert at the Colectiv nightclub just south east of the city center, and during the show some fireworks (pyrotechnics) were set-off which ignited a foam covered column that was adjacent to the stage.  Witnesses said the fire rapidly spread across the foam and pine wood covered ceiling causing a panic in the crowd.



Ironically, the band had just played 'This is the Day We Die' from their new CD "Mantras of War", and then as they started into the next song the fire started.  Delia Tugui, a Spanish teacher at the American International School of Bucharest who was at the concert with her husband and son, said club goers were surprised by how fast the fire spread and panicked.

"The lead singer made a quick joke: 'This wasn't part of the program.'  The next second, he realized it wasn't a joke and asked for a fire extinguisher," she wrote on her Facebook page.  "In 30 seconds . . . the fire spread all over the ceiling.  People rushed to the entrance but it was too narrow, and people panicked.  Behind me people stampeded, climbing over each other, to try and get out."

"I was two meters from the door and I barely got out," she told The Associated Press.  "People started pushing each other, stamping on each other, it was incredibly quick."

"Friends were looking for each other under the pile of people," she said. "It was a nightmare."

She said she knew some would not escape.

"I realized that those on the other side of the bar would not get out alive."

Once she got outside there was a blast and her hair caught fire, Tugui said.

"I tried to put it out with my hands and got burned.  People behind me were burned from head to toe."

The band’s lead singer, Alex Pascu, was hospitalized and later died.  Antena 3 TV said bass guitarist Alex Telea was reported missing by his parents, who have been searching for him.  he is also confirmed dead.

Florentina Dinca, 38, who lives opposite the nightclub, told The Associated Press there was chaos as emergency workers tried to aid the badly burned clubgoers.

"They were very frightened, they screamed, ambulances didn't know what to do," she said.  "Girls had their hair burned, they had burns on their faces.  It was terror."

“There was a stampede of people running out of the club,” a man who escaped without shoes told Reuters.

A young woman who sustained minor injuries described the club bursting into flames.  “In five seconds the whole ceiling was all on fire. In the next three we rushed to a single door,” she told television station Antena 3.

The nightclub had only one entrance / exit and it was just 80cm wide (31 1/2").  Paul Angelescu, a reporter with ProTV, said the mayor told him the venue had all the necessary permits to operate as a club, however, they did not have permits for any pyrotechnic use.  Further investigation will determine if the facility was indeed compliant with current local building codes.

The club was said to have 300-400 in attendance that night, and some were minors. It was only permitted for 80 occupants.  In Romania it is permissible for adults to bring their children into nightclubs, and occasionally unaccompanied minors enter the clubs because the monitoring of age is very lax.

The nightclub is one of dozens of clubs and bars that have sprung up in the Romanian capital in recent years, a lively city of 3 million renowned for its nightlife and long hours.

Shooting candles and indoor fireworks are common in bars and restaurants in Romania and enforcement of fire regulations is poor.

Twelve hospitals were filled with burn and smoke inhalation victims, and hundreds lined-up outside to donate blood.  The death toll is expected to rise.

Update:  Thousands of protesters in Romania have been gathered in Bucharest expressing anger towards the government for the lax building code enforcement that led to this tragedy.  Prime Minister Victor Ponta and his Cabinet resigned Wednesday, November 4 in response to public demands for accountability.

If this story seems all-to-familiar, numerous nightclub fires have been initiated by the illegal use of pyrotechnics which ignite foam materials used for sound absorption.  These include The Station nightclub fire in 2003 at West Warwick, Rhode Island, which killed 100 people; and and the Kiss nightclub fire in Santa Maria, Brazil, which killed 242 people.

Foaming at the mouth? Well, that's better than on the stage.

Foam materials are used in many different ways on the stage, and many times no forethought is given to the legality, safety, or potential consequences that accompany these choices.  Many foam materials are very combustible, and when they do ignite, they produce an enormous amount of black toxic smoke.  This is smoke so thick that it can kill you in seconds.  Need proof of this threat?  Here are three prime examples of recent fires that were all largely foam fire related:
At left is the pool deck of the Cosmopolitan Hotel in Las Vegas, Nevada.  You can see the fake palm trees that are sculpted from foam burning wildly.  NOTE THE AMOUNT OF BLACK SMOKE BEING PRODUCED.  If this was inside a theatre it would completely fill the stage house and audience chamber.

In each case, the primary source of fuel for the fire was foam based products, and in each case the smoke they produced was thick and deadly.  All too common is the use of foam insulation board like "Foamular" (see below) and "Rmax R-Matte".  This blogger sees it used for props and set walls with no attempt to enclose it or treat it with any fire retardant.  What are these people thinking!?


Also be aware that the NFPA 101 Life Safety Code specifically addresses foam materials uses in scenery and props in Article 13.4.6.11 Flame Retirement Requirements.  If you work in a theatre you should have a copy of this book.  It is fairly inexpensive and full of relevant information for theatre operations - particularly Chapter 13, but other Chapters and Annexes are very important, too.

In a minute . . you can lose everything

Bulwark, a manufacturer of fire retardant clothing, has commissioned a short film about the fragility of our existence.  The images in the film, along with the narrative, are well produced and thought provoking.  The film, 451°, is titled as a play on the famous Ray Bradbury novel Fahrenheit 451.  The intricate model constructed for the filming is similar to the scale stage sets frequently fabricated to conceptualize a project during the design phase.  Check it out!


Bulwark's products can be useful to those working in the performing arts in areas that involve pyrotechnics, open flames, or the risk of arc-flash exposure when working around electrical panels.

Burning the candle at both ends...(Updated)

When Disney's Imagineering group needed a spooky candle for their "Haunted Mansion", they couldn't be bothered with using real candles.  The fire hazard alone makes that a no-go, but as with all things in theme parks, the operational cost has to be evaluated, too.  Replacing real wax candles multiple times per day would be both material and labor intensive.  Using incandescent lamps wasn't a real good option either, as they consume lots of energy, have relatively short lives (by theme park standards), and still generate a lot of heat.  More heat means more air conditioning, means more electric bills.  Besides that, they just don't look like a candle - you know: flickering randomly, soft ethereal glow, and all that spooky stuff.

Well, if you can bring Abe Lincoln, and other animatronic things to life, why not a candle?  Add a little Disney magic to some LED's and a bit of creative molding for a candle body, and poof:  You have a Dream Candle.



First introduced at the 2010 CES show by Luminara LLC (originally Dream Candles, LLC), they have exclusive rights to commercialize Disney's patented idea.  They now have product available and information can be found at www.myluminara.com (this site redirects to a UK site, but if you go to www.facebook.com/pages/Dream-Candles-by-Luminara-LLC/415944677305 you can find US resellers.)

Don't use real candles on stage unless you can meet the requirements of NFPA Life Safety Code 101, Section  13.7.3.  If you do plan to use real candles, then you should obtain a copy of NFPA 160 Standard for the Use of Flame Effects Before an Audience.  This standard is referenced by the 2009 NFPA Life Safety Code 101, Section 13.7.3.(2).  Failure to know the requirements of the law is not an excuse to break the law.



There are other pretty good LED candles out there, like Smart Candle (http://smartcandle.com/), and some are even less expensive than real candles, and they don't drip hot wax on your actors, melt your props, or ignite your costumes that you failed to treat with fire retardant.  (Ref: 2009 NFPA Life Safety Code 101, Section 13.4.5.11 - Flame Retardant Requirements).

National Fire Prevention Week - Reducing the Fuel Load


So, what is this you are looking at? It's costumes (some neatly bagged in plastic) hanging from a batten over the stage. This type of storage should be discouraged for a variety of reasons:
  • It contributes to the fuel load on the stage should a fire break-out.
  • If the bags and synthetic materials catch fire, or even are exposed to fire, they will melt and drip molten plastic droplets onto anyone / anything that is below them.

Intumescent paint is another good tool for the scene and prop shops. This paint 'chars' on the surface and prevents fire from getting to the combustible material behind it. Here are some resources for this:

Natonal Fire Prevention Week - A look back at the Iroquois Theatre Fire

Much hasn't changed in the last 100 years - I still routinely find Fire Exits blocked, dysfunctional Fire Curtains, and flammable scenery, props, and costumes in every theatre I visit.  These were the very cause of the Iroquois Theatre fire. Think about it!

National Fire Prevention Week: October 5-9, 2009


In honor of National Fire Prevention Week I thought it would be good to take a look back at one of the worst Theatre Fires that ever struck this industry. The Iroquois Theatre Fire in Chicago was integral in forcing the development of improved Fire Safety Codes and Building Codes. Although the facility was largely constructed from fire retardant materials, the fire started with scenery that was not fire-proof, the fire and smoke containment system failed, and many of the emergency egress routes were blocked.  In just a few moments, hundreds of people were killed.  A true tragedy in every sense of the word.

For those that are inclined to learn from the past, and sadly, realize that many theatres today suffer from EXACTLY THE SAME CONDITIONS, please read through some of the material at the following links:




Does your Fire Curtain work properly?  Does it meet all the current codes?  When was it last inspected?  When was it last tested?  Where are your records?  Where are your spare parts?  Where is the wiring diagram for the motorized hoist (if it has one)?  If you don't know the answer to any of these questions, find out!

Burning Stones

This was a truly excellent design for a stone wall. From the audience seating it really did look like rocks in a great wall. Up close, I was able to see how it was made: Each 'stone' was made from a brown paper lunch sack with wadded-up newspaper inside. They were glued to a piece of corrugated cardboard, and lightly accented with Krylon enamel spray paint (I saw the empty cans). There were many of these wall panels. Guess what? Not a single part was treated with any fire retardant. One spark and the whole wall would have gone up in flames! Location: Elementary school stage.

NFPA Life Safety Code 101, paragraphs 12.4.5.11 & 13.4.5.11 specifically requires scenery and stage properties ('props') to be constructed of noncombustible materials.

"Allright Bugsy, it's curtains for you."

Drapes and other soft goods used on the stage must be treated with a chemical fire retardant or be fabricated from an inherently flame proof material. When the manufacturer creates the product they should tag it so that there is a record of the date of manufacture / treatment, and retreatment, if any. The label shown above is a good example of that type of marking.

So, what's wrong with this picture? The date of manufacture is 9-15-76. This picture was taken in March of 2009, and the curtain was still in use! 33 years later!

The chemical fire retardant treatment applied to theatrical fabrics is typically only good for 2-5 years, depending upon the heat and humidity that the fabrics have endured over their service life. Soooo, this curtain was, shall we say, waaay past it's useful working life.

Check your curtains, see that each one has a serial number or unique ID marker, has a tag showing date of manufacture (not date of installation or first use), and lists the name of the chemical treatment that was applied.

Keep separate records that corrispond to each soft good so that you can schedule and budget for periodic cleaning and service. This is a great use for any computer based appointment or calander system (like MS Outlook). Enter the date of manufacture as an 'appointment', and then set the recurrance for every "X" years (as recommended by the manufacturer). The computer will remind you of your appointment when it rolls around, just like a birthday or a dentist appointment.
