Better Ladders - Safer Work

You've all seen it - the late night infomercial for the Little Giant Ladder.  You may even have one for getting to some of those difficult to reach places.  This year at LDI, these guys outdid themselves BIGTIME.  They introduced a extensible A-Frame ladder with a safety cage work platform at the top.

"Alas, the old trestle ladder, 'Rickety' was unceremoniously taken out to the dumpster, cut to pieces with a chain saw, and laid to rest.  A few onlookers mourned his passing, as they held-back signs of PTSD from sitting atop it with their legs entangled in the rungs while they wrangled a twenty pound ellipsoidal light into position.  Splintery's friend, 'Skateboard' was also cut to shreds and tossed into a cold, dark dumpster to be forever forgotten.  Even his wobbly, non-locking casters were committed to the recycle bin in a final tribute to stupidity.  The local laundromat is expected to see a sharp decline in stained underwear noted one person.  "For every inch the trestle ladder shifted unexpectedly while you were on it, there was a proportional brown streak in my whitie-tighties" they reminisced."
Don't be fooled by the picture at the right -- the new CAGE ladder can extend the working platform height to 13'-4", providing a reach range to 19'-10".  This can get you up to many electric battens and pipe grids while providing a easy to move, stable work platform.  Imagine being able to SAFELY use BOTH HANDS AT ONE TIME.  What a radical concept for a ladder!

Notice the wide stance, too.  And the best part is that ONE PERSON can move it around.  The fiberglass frame is electrically non-conductive while providing a strong structure that is lightweight like an aluminum ladder.

Ladder Safety now a smartphone application

NIOSH has released its first smart phone application (app) for mobile devices.  This free app is aimed at improving extension ladder safety by providing real-time safety information delivered via the latest technology.  Falls from ladders are a persistent source of workplace injuries.

Misjudging the angle at which an extension ladder is set-up is a significant risk factor that can be reduce and prevent falls. If an extension ladder is set too steeply, it is more likely to fall back or slide away during use, and if it is set too shallow then the bottom can slide out. The NIOSH Ladder Safety phone app has an angle of inclination indicator which uses visual and audible signals making it easier for workers and other users to set an extension ladder at the proper angle of 75.5 degrees.

The NIOSH Division of Safety Research tested and patented the concept of the app’s inclination indicator. They compared existing ladder positioning methods and found that the indicator improved both the accuracy and efficiency for extension ladder positioning. The Ladder Safety app also includes other information about ladder safety including ladder selection, inspection, accessorizing, and use. While it may seem obvious, we have to say it: for your safety, use the app to set the proper ladder angle before climbing an extension ladder.


 The app can help workers prevent falls and is also a tool for employers who want to ensure a safe workplace. The app is available through the NIOSH website, the Apple App-storeExternal Web Site Icon, and Android Apps on Google PlayExternal Web Site Icon. It works with most SmartPhones that have inclination sensors.

Violinist Dies After Fall Into Orchestra Lift Machinery Pit

2013-07-16 - Moscow, Russia - A 40 year veteran of the Bolshoi Theatre died Wednesday at a Moscow hospital as a result of injuries sustained from a fall into the machinery pit of the Orchestra Lift.  Violinist Viktor Sedov, 65, was found lying on the pit floor about 2:30PM Tuesday when a theatre employee making a routine fire safety round found Sedov lying unconscious on the pit's concrete floor.  He was taken to the Sklifosovsky Hospital where he passed away the following day.

The accident happened in the New Stage venue, one of four venues in the Bolshoi Theatre of Russia complex.  The Main Stage venue had recently gone through significant architectural modifications, and it is suspected that Sedov entered the unlit sub-stage entry platform behind the orchestra lift and stepped out in the darkness to where he believed the Orchestra Lift Platform was positioned.  In the old configuration of the facility the platform was at the same level as the bottom of the pit.  The lift platform was up at a higher level and he plummeted into the machinery pit.  One report says the fall was about 6 meters (19 feet), but this may be the total depth from the stage, and not the depth from the sub-stage entry platform, however, another news report says that he fell from the stage level. Part of the facility reconstruction involved the addition of many levels of subterranean storage and workspaces, so it may be that the Orchestra Lift pit did extend down 6 meters from the substage platform.

On Wednesday, the Bolshoi's press secretary, Katerina Novikova, said the cause of Sedov's death will be determined by a special commission.  A decision regarding further legal action will be made depending on the outcome of the police inquiry into the incident.

Commentary

In this accident, there are several causal factors to consider:
  • Inadequate illumination of the walking surfaces.
  • Lack of guardrails to protect workers from falls.
  • Lack of interlocks on doors leading to hazardous workspaces.
  • Solo workers in unobserved workspaces.
We can learn from other industries' Best Practices and incorporate those procedures into our daily regimen.  In the sport of SCUBA diving they use the Buddy System - participants NEVER dive by themselves - there is always someone with you nearby that can you rely upon to help you if you need it.  Theatre facilities should have a similar policy.  There are 1000's of scenarios where a single person in a theatre can be injured and not be able to call for help.  If you have a co-worker with you the odds of both person's being incapacitated are significantly reduced.  Simply put:  Don't go into the theatre by yourself!

Cirque du Soleil suffers tragic loss of performer during show accident

MGM Grand, Las Vegas, Nevada - Saturday, June 29, 2013.   In a yet-to-be-explained equipment failure, one of the aerial performers in Cirque du Soleil's fell about 50 feet from above the show's massive 3-axis moving stage into the forestage pit during the fight scene at the end of a performance.  The pit is nearly 4 stories deep.  Sarah Guyard-Guillot, 31, mother of two, came loose from her safety cable and disappeared into pit unexpectedly.  The show was halted, the audience informed that they could obtain replacement tickets for a future show, and were dismissed.  The show has been suspended indefinitely pending an investigation into the root cause of the accident.  Guyard-Guillot, mother of two young children, was pronounced dead at a hospital late Saturday night.  It is reported that her safety harness was intact with her costume when she was recovered from the accident site.  She was a trained acrobat and aerialist, originally from Paris, France.  She had performed in the industry for about 22 years.

This is the first performance-related death that the troupe has experienced in over 30 years of show production.*  Guyard-Guillot was one of the original performer's in the show when it opened in 2006, and had performed the show since then.

An extremely through and rigorous safety program is entrenched in the companies' culture, so this has been shock to those that work the shows in a tight-knit group.  Cirque's safety managers have been very transparent and informative by sharing their safety culture and knowledge at numerous industry events like the annual USITT convention, LDI convention, and UNSCA's Entertainment Innovation Conference.

The death comes at an unfortunate time for the circus, a reportedly $1 Billion (US) company which employs 5,000 people globally, including 2,000 at its headquarters in Montreal, Canada.  Cirque maintains around 19 touring productions in operation worldwide, yet has recently reduced it's workforce and is trying to reign-in its expenses.

Cirque du Soleil's founder, Guy Laliberté, said in a statement:  "I am heartbroken. I wish to extend my sincerest sympathies to the family. We are all completely devastated with this news.  Sassoon was an artist with the original cast of KÀ since 2006 and has been an integral part of our Cirque du Soleil tight family. We are reminded, with great humility and respect, how extraordinary our artists are each and every night. Our focus now is to support each other as a family. "

* In 2009, a Cirque performer died in Montreal after sustaining head injuries from falling off a trampoline during training. Oleksandr Zhurov, who was 24, hailed from the Ukraine and was known as Sacha. There also have been several injuries during Cirque performances in the last decade. Actors have been hurt during performances of "Zumanity" in Las Vegas, Nevada; "Corteo" in Portland, Oregon; and "La Nouba" in Orlando, Florida.

The same night as the deadly fall during , Cirque's newest show, "Michael Jackson: ONE," held its opening night performance at the Mandalay Bay Resort and Casino in Las Vegas. That production experienced its own trouble the previous week.

On Wednesday, June 26, 2013, Cirque reported that an acrobat taking part in one of the final preview performances suffered a mild concussion after slipping through slack rope in the show's "Stranger in Moscow" scene. That performer, who is expected to return to the production, missed a protective pad below the actors and landed hard on the stage.

OSHA FINES CIRQUE DU SOLEIL AND MGM GRAND
FOR PERFORMER'S DEATH AND OTHER CONCERNS

2013-10-29  - Nevada OSHA has finished its investigation into the death of Sarah Guillot-Guyard, the Cirque du Soleil performer who fell 94 feet during the climactic scene of Ka on June 29, 1013 and later died.  OSHA determined that accident happened when the wire rope she was suspended from broke. According to the report, the wire rope “was severed due to the rapid ascent of the performer, ultimately causing the rope to be freed from the sheave/pulley and scraping against a shear point.”  OSHA officials cited Cirque du Soleil with six proposed citations and more than $25,000 in fines and MGM Grand with three citations and a $7,000 fine.  Cirque du Soleil and MGM are appealing the fines.

One of the citations for Cirque du Soleil includes a failure to “protect or prevent Ka employees from striking an overhead grid during the Ka show at the Ka Theater.” Reporting for the Las Vegas Sun, John Katsilometes wrote that “This matches reports from those familiar with the Ka staging and fatal incident that Guillot-Guyard did hit the metal grid over the stage as she ascended high above the Final Battle scene, which jarred the rope connecting her to her harness.” Katsilometes also reported that Cirque du Soleil will be appealing the fine:

    In a statement that also referenced Cirque’s internal review of the incident, [Cirque du Soleil spokeswoman Rene-Claude Menard] said: “Cirque du Soleil completed an exhaustive review of its safety policies and procedures in the wake of the tragic accident involving Sarah. We have redoubled our efforts to ensure the overall diligence and safety of our performers and crew. We have received and reviewed the OSHA citations. We have initiated the appeal process as part of OSHA’s administrative protocol. Safety always has been the top priority for Cirque du Soleil, its performers and crew members.”

Cirque du Soleil:
- NRS 618.375 (1): A General Duty Citation was issued because the employer did not protect or prevent KA employees from striking an overhead grid during the KA show at the KA Theater. $7,000 proposed penalty.

- The first part of the General Duty Citation was issued because the employer did not provide proper training for the KA Battle Spearman Warrior employee involved in the accident in the use of equipment and tasks used in the Ka show, battle scene at the KA Theater.

- The second part was issued because the employer had a Fall Protection Program with construction requirements that do not apply to theater settings because the KA Theater is regulated by Nevada OSHA general industry standards. $7,000 proposed penalty.

- 29 CFR 1910.132: This citation was issued because the employer did not properly assess the workplace for hazards that required personal protective equipment at the KA Theater, including opensided floors, bloodborne pathogens and other potentially infectious materials, pyrotechnic dust cleanup, and not finalizing a hazard assessment for the performers. $7,000 proposed penalty.

- 29 CFR 1910.132: This citation was issued because the employer did not certify that a workplace hazard assessment had been performed and did not include the date the hazard assessments were conducted. $0 proposed penalty, grouped with the proposed citation above.

29 CFR 1904.32: This citation was issued because the employer did not include on the OSHA Form 300 Log of Injury and Illnesses, the object or substance that directly injured or made a person ill, which prevented a trend analysis and kept the employer from recognizing and conducting employee training involving recurring injuries in accordance with Nevada Revised Statute. $3,300 proposed penalty.

Nevada Revised Statute 618.379: This citation was issued because the employer removed equipment from a fatality site on June 29 before Nevada OSHA authorized the dismantling and removal of the equipment (38 feet of wire rope that was attached to the victim at the time of the accident). $935 proposed penalty.

MGM Grand Hotel & Casino:

- NRS 618.375: This General Duty Citation was issued because MGM Grand employees were exposed to hazards due to deficiencies in the Cirque du Soleil Fall Protection Program with construction requirements that do not apply to theater settings because the KA Theater is regulated by Nevada OSHA general industry standards.

- 29 CFR 1910.132: This citation was issued because MGM Grand employees were exposed to hazards due to the deficiencies in Cirque du Soleil hazard assessments for the KA Theater that included opensided floors, bloodborne pathogens and other potentially infectious materials and pyrotechnic dust cleanup. $7,000 proposed penalty.

- 29 CFR 1910.132: This citation was issued because MGM Grand employees were exposed to hazards due to deficiencies in the Cirque du Soleil hazard assessments because Cirque du Soleil did not certify that a workplace hazard assessment had been performed and did not include the date the hazard assessments were conducted. $0 proposed penalty, grouped with the proposed citation above.

Fall Into Orchestra Pit Claims Another Life

On March 17th, 2013, the Acting Chief Director of the Perm Opera and Ballet Company backed-up and fell about 3m (~10 feet) into an open Orchestra Pit during a rehearsal of Peer Gynt.  Olga Enns, 33, struck her head and was urgently rushed to the hospital where doctors assessed her injuries and said she here chances of survival were minimal.

Olga spent the next two week in a coma, and on April 2, 2013, she passed away.  She is survived by here mother and a sister.  Olga had worked at Perm since 2006 and was integral in the effort to make  the theatre more friendly to visiting performance companies.

OSHA Campaigns for Ladder Safety

The US Occupational Safety and Health Administration unveiled a new bilingual English-Spanish booklet, "Falling Off Ladders Can Kill: Use Them Safely."  The short guide, part of OSHA's national campaign to prevent fatal falls — the leading cause of  death  in construction — provides clear, easy-to-follow information about ladder hazards and how to use ladders safely, featuring illustrations and plain language writing.  The falls prevention campaign now enters its second year and OSHA is working with the National Institute for Occupational Safety and Health (NIOSH) and the National Occupational Research Agenda on outreach.

Arena Rigger fails to clip-in and falls 100 feet

Friday, April 5, 2013 - San Antonio. Texas.  A high steel rigger fell to his death at the AT&T Center Arena while striking a show at 2:00 AM.  Witnesses said he had detached himself  from the fall safety line while repositioning himself and lost his footing while moving around a beam.  A late load-out can be dangerous as fatigue can play a factor in clarity of thought and situational awareness. 

The rigger was identified as Dean Williams, 44, from Houston, Texas.  He leaves behind a wife and 3  month old daughter.

Accidents like this can be avoided if you wear a dual (twin) lanyard tie-off :

If this is not convenient, then you might consider using a dual retractable fall arrestor like this:

Either style works well and  can provide good maneuverability while being tied-off.  Always have a rescue plan when working at height.  Hanging in a fall protection harness for even a short period of times can result in suspension trauma if you cannot relieve the pressure on your thighs from the harness straps.

Bucket Bails Break Bones

The ubiquitous plastic bucket - it shows-up unannounced, hangs around like an unwanted house-guest, and finally spills it guts and gets thrown in the trash.


Aside from the obvious bad rigging (you did see that, didn't you?), plastic buckets have a serious weak point:  The metal bail that forms the carrying handle.  This is nothing but a piece of bent steel wire that hooks into the plastic rim of the bucket.  Given sufficient (over)loading, the plastic bucket can deform and let-go of the bail (literally tearing the plastic), and/or the metal bail can bend and lose it's grip on the bucket.  Either way - this thing is going down.

We just love to over-stuff our carry-alls, and when you add-up the weight of hand-tools, nuts & bolts, drills, and all that other crud you forget to clean-out, the weight can become significant.  This wouldn't be such a big deal if you were just carrying this across the stage or shop, but there are times we forget logic and tie a rope on thee a try to hoist them up to elevated work locations.  Not such a good idea - What was once just going to fall  a few inches onto your foot now becomes a bomb falling from overhead - maybe 20 feet, maybe 50 feet, depending where you are going with it.  Either way, this can be deadly should the handle fail.

Safe lifting of materials is usually best accomplished when the lifting system encompasses the load, this places the load on the lifting device and not on the object being lifted.

The folks at The Circus Company (I guess circuses use these things to haul food and water for the animals) have a simple, yet effective solution:  Build a load-rated sling designed to fit those 5-gallon buckets we are so fond of.

These slings are rated for 200 Pounds and are inexpensive (relative to the cost of a new noggin').  The handle is constructed of reinforced Nylon, and has an extra layer across the hoist point.  It's pretty snazzy, too!

Shackle Pin Safety

When working at height, even the smallest of dropped objects can become deadly projectiles.  Show riggers working in arenas and stadiums can sometimes be more than 100' above the floor, so even a shackle pin can achieve a terminal velocity that could penetrate a hard-hat.  Ty-Flot, maker of all things to do with securing your tools, has announced the availability of a safety lanyard for shackle screw-pins.  Set-up with a mini-pivoting joint in the cable, it allows the free rotation of the shackle pin for installations all-the-while keeping it safely attached to the shackle bow when disengaged.  The device quickly installs using one field crimp.

Going Down? Self-Rescue Beats Hanging Around

The US OSHA and UK HSE Fall Protection standards include a requirement that a rescue plan be in-place before climbing operations begin.  Calling 911 is not a rescue plan.  When a worker is hanging in a fall protection harness they can be subject to suspension trauma, so getting them down to terra-firma (the more firmer, the less terror) is the first priority.

Suspension trauma is the shut-down of the circulatory system due to the constrictions placed on the body by the fall protection harness.  There are kits that you can supplement on your climbing gear that will deploy a short set of bootstraps that tie back to the suspension D-Ring and will allow you to 'stand-up' and take some of the pressure off of your leg and waist fittings, but this is only a temporary solution to the problem.

The OSHA / HSE rescue requirements allow for self-rescue.  This can be a real time saver, as getting to a rescue kit, taking it up to an attachment point above the fallen worker, and reaching or rappelling down to attach the kit, then lowering the fall victim to the ground can be very time-consuming.  Here are three self-rescue solutions that are worthy of consideration:

The Latchways PRD (Personal Rescue Device) is a backpack module that is fully-integrated with the climbing harness and preconnected to the fall arrest lanyard.  If you fall, you just reach up to your shoulder and pull a release cord that unlocks the descender and it lowers you to the ground.  Current models have a 65 foot rescue rope, and longer options are forthcoming. Working At Height is a Complex Issue

Event Industry News published an interesting article about Working at Height.  This is something that is almost always required during the mounting, running, and strike of most events.  However, the definition of what ‘working at height’ actually is should serve as a reminder to organizers of the important role played by event safety consultants.  Sadly, most events in North and South America don't have independent oversight of personnel safety, they tend to lump the task in with the event security work and fail to see the difference between the two roles. No show is so important that workers should ever be put at risk, yet they are routinely asked, nay, TOLD to do things that place riggers and ground crews unnecessarily in danger of injuries, or worse.

Chris Hannam from the UK safety consultant firm Stagesafe says that some of the myths surrounding health and safety are absurd, often sensationalized by the media and driven not by the UK Health and Safety Executive, but by insurance companies.  He says that it’s important not to be mislead by incorrect health and safety myths, but instead to keep things simple and read the correct guidance.  In his column, Chris provides the essential guidance for event professionals when working at height.

Actor accidentally really hangs himself.

The celebration of Easter frequently has re-enactments of significant events in the life if Christ.  In this case, an Brazilian actor, Tiago Klimeck, 27, was recreating the suicide of Judas Iscariot as he hangs himself as described in the Bible in the book of Matthew.  This is part of a production of the play "The Passion of the Christ."  The event was presented on Good Friday in the southern city of Itarare, Brazil.

Klemick was wearing a fall protection harness and allegedly knew how to arrange the equipment under his robe costume so that the rope around his neck would not cause him any harm.  He had borrowed the harness from the local Fire Department on previous occasions, so they did not review the donning of the equipment with him.  Witness say he was hanging nearly four minutes before anyone became concerned.

Luiz Carlos Rosner owns a sandwich cart next to the city square where the play was taking place. He described the anxiety after cast members realized Klimeck was unconscious.  "One of the actors came over to me, desperate, explaining there was someone unconscious hanging from the rope and that he wanted to cut it," Rosner told TV Record. "I was a little worried about giving him a knife in the middle of the crowd."

He was taken down and rushed to the hospital where he remained in a medically induced coma in a medically induced coma for 16 days due to extensive brain injuries from a prolonged lack of oxygen.  He passed away in he Hospital Santa Casa de Misericordia, in the neighboring city of Itapeva.

The harness, along with the rope used in the play, are now being analyzed at the Criminal Institute of Sorocaba.

No matter how confident performers and technicians are in setting-up stunt work, it is vital that pre-show rehearsals review the effect, the equipment, and the adjustments of the components to ensure performer safety.  It is also important to have a quick way to check if performers are in trouble.  In this situation, a discrete hand signal that would only be possible if the performer is conscious might have provided just enough warning to  a spotter that the effect had been compromised.  Ideally, the harness and rope system would have been reviewed to ensure that the relative tensions were proper.  Presumably, there was someone there to lower the actor to the ground after the scene, and this person should have had eye-contact and/or intercom communications with spotters, too. The search for a knife to cut the actor down indicates that there was no easy way to lower the performer to safety.

NO stunt is worth your life.  Plan, check, certify, evaluate, think of all the ways the stunt might go wrong, re-evaluate, check again, test it, and re-test it.

Ladder Safety Should Be A High Priority

Ladder Safety is an ongoing concern in the performing arts as more and more venues are constructed with inadequate access to the working parts of a production facility. Powered Manlifts are one possible solution, yet many administrators somehow think that they are less expensive than a trip to the hospital.  Even with a Powered Manlift, ladders are frequently found around many workplaces because they are a convenient way to access many items

Make a concerted effort to train students, co-workers, and administrators about Ladder Safety so that there is a consistent perspective on the subject throughout your organization.

Santa's Elves need to keep the workshop floor clear of obstacles

An elderly lady accompanying five members of her family to Santa’s grotto at a well known department store in London was injured during her visit, and the jury (not a team of reindeer) decided in her favor.  She successfully sued the events management company organizing the festive treat blaming Santa and his elf for failing to prevent her fall.

The grotto was operated by two employees, one who played Santa and the other an elf.  The elf escorted visitors in and out of the grotto whilst Santa sat on his customary throne entertaining his guests.  The elf and Santa both stated that they checked for loose items on the floor.  The injured lady's case was that she had lost her balance by stepping on a plastic icicle on the floor of the grotto which should have been seen and removed.

Section 2 of the UK Occupiers’ Liability Act of 1957 required the events company to see that visitors would be reasonably safe in visiting the grotto.  This was found to not be the case.  The elf was concerned with many other duties – at a guess – toy-making and feeding reindeer perhaps!  It was found that Santa and the elf were not as careful in checking the floor as they should have been. If the icicle was there to be stepped on, it was there to be seen.

The overly benevolent view of the performance by Santa and the elf of their duties at first instance was overturned by the Court of Appeal who allowed the claim. Their checking of the grotto floor was held inadequate in protecting the public from falls.

With claims against events holders attracting increasing publicity, this case highlights that the bar is set high for those responsible for carrying out hazard spotting.  Some guidance can be gleaned for events managers from the facts that have been touched upon in this case.  The main problem the events company could not overcome was the concern that time demands on their staff limited their attention towards taking precautions.  Staffing should be suited to the event in order to demonstrate that sufficient time and regard has been given to periodic checks, ideally being documented, as part of a documented risk assessment.  A higher standard of inspection is demanded for smaller areas such as this grotto, particularly in the context of the poor lighting present.

Employees need to be extra vigilant at all times, even in the season of goodwill to all men.

Be on the lookout for reindeer poo, too!  That could make you slip and fall off of the roof.

OSHA releases videos in the wild - hopeful that no one will be killed or injured

OSHA animated educational videos show how to protect workers from construction hazards.

WASHINGTON (2011-11-14) – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has released 12 educational videos about potential hazards in the construction industry. The educational videos are easy to understand, short segments and geared to employers and workers. Each year, nearly 800 construction workers die on the job; one in every five workplace fatalities occurs within the construction industry. The videos are based on real-life incidents and include detailed depictions of hazards and the safety measures that would have prevented these injuries and fatalities.

"I urge anyone who works in the construction industry or operates a construction business to watch the videos. Share them with your co-workers and friends in the construction industry; organize screenings for your workers; and post them to your web pages," said Assistant Secretary of Labor for Occupational Health and Safety Dr. David Michaels. "Every step we take to educate workers about their rights and the safety measures employers must take to protect workers in construction helps us avoid preventable injuries and the tragic loss of life."

These videos cover falls in construction, workers who are struck by vehicles and heavy equipment, sprain and strain injuries, trenching and excavation hazards, and carbon monoxide poisoning. These videos are written for workers and employers, including workers with limited English proficiency.

Most of the videos are two to four minutes in length, and all but one are animated. Each video is available in English and Spanish for Web viewing or downloading. All video scripts are also available online in English and Spanish. Use and Selection Guides for Fall Protection PPE from ISEA

Two new documents from the International Safety Equipment Association (ISEA) will help protect workers whose jobs expose them to fall hazards.  A Personal Fall Protection Equipment Use and Selection Guide provides practical, hands-on guidance for fall protection users and administrators in their selection, use, maintenance and inspection of fall protection equipment.  A companion document addresses topics in fall protection on which manufacturers get frequent inquiries.

Prepared by manufacturers in the ISEA Fall Protection Group, the use and selection guide describes the process of developing a corporate fall protection program, explains the components of fall protection systems, gives examples of how to select equipment for various types of work, and outlines steps for planning the use of fall protection systems.

The guide also contains inspection and maintenance guidelines, definitions, a list of applicable OSHA regulations and US and Canadian consensus standards, and links to ISEA companies and other sources of information.

“Falls are a leading cause of death and injuries in the workplace, and they’re preventable,” said Bob Apel of MSA, chairman of the Fall Protection Group.  “At many work sites, fall hazards can’t be eliminated, and ISEA members make the best equipment and systems in the world to prevent and stop falls.  This guide will help employers and users understand how to select the right equipment and use it properly.”


The second document, Frequently Addressed Topics in Fall Protection, is the first in a new series of “PPE Perspectives” papers from ISEA.  It answers some of the most commonly asked questions that fall protection manufacturers are asked about equipment and systems, applications, and other considerations in planning and implementing a fall protection program.

Equipment issues include anchorage strength and location, horizontal lifelines, harness attachments, positioning of self-retracting lifelines, twin-leg lanyards and tie-back applications.  There are discussions of the importance of ensuring compatibility among components and connectors in a fall arrest system, determining the service life of equipment, planning systems to protect heavy workers, and post-fall suspension.  Specific application topics include welding, residential roofing and aerial lift devices.

Both documents are available online in PDF format, so that they can be read online, downloaded to computer or portable device, or printed.  They are available free of charge, and will be updated as required by changes in regulation or standards.

Weebles Wobble - But They Don't Fall Down

What better time to talk about Ghost Lights than during the Halloween season?  One of the prime reasons to have a Ghost Light on your stage is to prevent accidents.  Stages have places you can fall off of like trap openings and the front of the stage, and they have plenty of things to bump into and trip over, or at least bust-up your face or shin real good.  Ouch!  Halloween scars that are permanent.  Just what every aspiring actor needs.

One of the most common reasons for not having a Ghost Light is that they aren't commonly available from theatrical supply houses.  All of the Ghost Lights I've ever seen have been cobbled together by well meaning shop craftsmen. The problem with this approach is that whenever you manufacture something you assume the liability for it as well.  When you combine this with poor electrical knowledge and make-shift adaptations the results can be electrically hazardous as well as physically hazardous.  So where is the safety in that?

I was at the local big-box hardware store the other day and I noticed this gizmo:

Lowes sells it under their Utilitech brand (made by Cooper Lighting), it is UL and CSA safety listed, fairly inexpensive, and most of all, pretty darn indestructible.  The unit has a weighted base that houses the fluorescent lamp ballast, and incorporates a groove around it to wrap-up the cable when it is not in use.

The case is plastic and has a tie loop at the top if you need to hang it up for storage or suspend it. The clear plastic tube that house the lamp acts as a container to keep broken glass off of the stage should you give it a really significant whack.

Fortunately, the unit does NOT come equipped with a lamp (yes, this is a GOOD THING).  When you outfit it with a lamp you can choose the color temperature (CCT) and color rendering index (CRI, or Ra) so that it doesn't clash with your other work lighting, and if you elect to beef-it-up a bit you can install a hard-use lamp like those made by ShatRShield (www.shatrshield.com/catt8page.html)

This unit uses less than 40 Watts of power and lights-up a pretty good area around the stage, so pop one out onto the proscenium line so nobody has to grope in the dark and fall off into the orchestra pit.  That's a dangerous fall to make unexpectedly, and many people over the years have been killed or received career-ending injuries this way.

And a note about the Energy Nazies:  There is always some well-meaning but oblivious-to-the-safety-implications individual that feels compelled to turn-off every light they find glowing.  For a  Ghost Light  to be effective, it must be left ONYou have to twart them at their game.  What can you do?  Post a sign at both the on/off switch on the Ghost Light AND at the end of the cord(s) that supply the power.  Be polite, but be firm.

 "I want you to meet my little friend." said Al Pacino's character in Scarface.  This will confuse the energy Nazies for sure:  Add a  photoluminescent  tube over the lamp.  This will continue to glow for about 40 hours after you remove the power from the lamp - so it will give any stage marauders a bit of a chance see where they are going should the power go off (or be switched off).  The Lumenite® Light Sleeve is a semi-translucent,  photoluminescent  sleeve that fits readily over new or existing fluorescent tubes.

Do you really want to keep it safe and confuse the Energy Nazies?  Add an Uninterruptible Power Supply (UPS) to the Ghost Light.  At only 40 Watts power draw, a small UPS will keep the lights on for hours.  If you use an LED based lamp, then the darn thing could glow for a day or more.
